California Department of Forestry and Fire Protection’s (CAL FIRE’s) Electronic Employee Medical Questionnaire

You have been directed to the California Department of Forestry and Fire Protection’s (CAL FIRE’s) Electronic Employee Medical Questionnaire (E-EMQ).

The information you enter on the following pages is confidential. A copy of your responses to the E-EMQ will be maintained in your medical file, which is separate from your Official Personnel File. Protection of your personally identifiable information is a shared responsibility. CAL FIRE has taken measures to safeguard your information through security features built into this application. Access to your health information is limited to CAL FIRE staff directly employed in CAL FIRE’s Medical Assessment Unit who fall into a “need to know” category based on the duties they perform in the medical evaluation process and to the CAL FIRE Medical Consultant. As you submit your information, the data is encrypted. In order to maintain the system, the contractor (UC Davis) has access to the encrypted data; they are bound to maintain confidentiality under the terms of the contract.

You are not to share your user name and password with anyone, including supervisors, co-workers and family. Neither your supervisor nor management may look at or review your answers. We also urge you to protect any data you opt to print or capture via screen shot to prevent it from falling into the wrong hands.
